Not An Allergen … yet

Anecdotal reports of reactions to stevia — contact or IgE-mediated — are so rare that it is not normally included in standard patch test trays. Being a flowering plant from the Asteraceae (compositae) family, however, it would not be surprising for this increasingly popular sweetener to become a top allergen in the future.

Many plants and herbs in the compositae family are top contact allergens and top IgE-mediated allergens. This group includes daisies, sunflowers, feverfew (chrysanthemum), and asters, plus chamomile, arnica, and over 20,000 other herbs, vegetables, and plants. Of particular concern in this group are sesquiterpene lactones, which are found in the stems, pollen, flowers, and leaves. Stevia is a flowering plant (an herb) in the aster family that’s also called “sweet leaf” — its sweet-tasting leaves contain the steviol glycosides (stevioside is an intensely sweet diterpene glycoside).

Most patients with a contact allergy to sesquiterpene lactones seem to also be allergic to Asteraceae-derived herbal teas (especially chamomile, dandelion, and wormwood). Containing sesquiterpene lactones, it seems likely that as stevia continues its explosive popularity (the more commonplace a substance, the higher the risk of it becoming an allergen), it may become a top contact allergen.

That said, if you haven’t patch tested positive to sesquiterpene lactones or compositae mix, you might be able to enjoy stevia, which does have important health benefits! Around 300 times sweeter than sugar but without affecting blood glucose levels (non-glycemic), stevia is widely considered to be a healthier alternative to sugar.
